Here are forms that you will need for your annual paperwork for NMFMA. They are all PDFs that can be typed directly into and emailed back to heidil@FarmersMarketsNM.org. You may also mail them to NMFMA, 731 Montez Pl., Santa Fe, NM 87501 or fax to (505) 995-1043.


Instructions for downloading and completing forms
1. Click on the link to the form to open and download
2. From the FILE menu, click on SAVE AS
3. Rename the file and save to your hard drive
4. Fill out the blanks in the form by typing directly on the lines. If there is a set of circles, click the one that corresponds to your choice
5. From the FILE menu, click SAVE to save your changes
6. In your email program, compose a message to heidil@farmersmarketsnm.org and click on ATTACH
7. From the search menu, click on the file that you renamed and saved
8. Send email
